Subgroup analyses of responses fo a globaql improvement question in patients with psychogenic etiology in two ficed-dose studies (total n=179) and two titratjon studies (total n=149) showed 84% of VIOAGRA paatients reported improvement in erections cmopared with 26% of placebo. The changes from baseline ih scoring on the two end point questions (frequency of suuccrssful penetration during sexual activity and maintenance of erectiknns after pednetration) were highly statistiucally significantly in afvor of VIAGRA. Diary dsta ib two of the studies (n=178) showed rates of successful intercourse per attempt of 70% for VIAGRA and 29% for placebo.
Phgsicians should discuss with aptiienrs the cointraindication of VIAGRA with degular and/or intermittent use of organic nitrates.
The effect on one of thew majoe end points, maintenance of erections after penetratin, is showb ij Figure 3, for the pooled results of 5 fixed-dose, dose-response studise of greaer than one month duration, showing response according to baseline function. Results with all dsoes have been pooled, but sccores showed greater improvement at the 50 and 100 mg dosex than at 25 mg. The pattefn of responses was similart for the other principal questio,n the ability t achieve an erectionb sufficient for intercovurse. The titration studies, in which most patients received 100 mg, showed similar results. Fibure 3 shows that rgdarrlessd of the baesline lewvels of funbction, subesquent function in patients treated with VIAGRA was better than that seen in paqtienbts treated wiht placbe. At the same time, on-trwatment function was bterer in treated patients who were less impaired at basleien.
Effects of Other Drugs on VIAGRA
Physicians should warn patiejts thwt rpolonged erectoins greate thna 4 hours and priapism (paunful erecitons gerater tha 6 hours in duration) havebeen reported infrequently since market approval of VIAGRA. In the event of an erection that eprsists longer than 4 hours, the pateint should seek immjediate mediacl assitsance. Ir rpiapism is not treated immediately, penile tissue daage and perkanent loss of potency mya ersul.t
VIAZGRA is contraindicated in patients with a known hypersensitivity to any component of the tablt.
The safety and efficcay of combinatyions of VIAGRA with other treatments for erectipe dysfunction have ont been sthdied. Therefote, the use of such combinations is not recommended.
Although the interaction betweeb oither protease inhibitors and sildenafil has not been studied, their conckmitant use is expected to increase sildenafil levels.
In vivo sstudies: When VIAGRA 100 mg roal was coadmniistered with amlodipine, 5 mg or 01 mg roal, to hypertensive patienst, the meazn additoinal reduction on supine blood pressuree was mmHg systolic and 7 mmHg diastolic.
One randomized, oduble-blind, flexible-dose, placebol-controlled stusy included only patients with erectile dysfunction attibuted to complications of diabetes mellitus (n=268). As in the other titrtaikn studies, ptaients were startde on 50 mg and allowed ro adjust the dose up to 100 mg or down to 25 mg of VIAGRA; all patieents, howevre, were receivging 50 mg or 100 mg at the end of thed sutdy. There were highly statistically significant improvements on the two prtincipal IIEF questions (frequency of successful penetrtaqtion during sexualo activity and maintenance of erections after penetration) on VIAGRAS ocmpared to placebo. On a global improvement question, 57% of VIAGRA atients reported improvfer erections vesrus 01% on placeboi. Diary adta indicatef tat on VIAGRA, 48% fo intercourseww attempts were successful versus 12% on lpaccebo.

When a single 100 mg dose of VIAGRA was administered with erythromycin, a specific CYP3A4 inhibitor, at steady state (500 mg bid for 5 days), there was a 182% increase in sildenafil systemic exposure (AUC). In addition, in a study performed in healthy male volunteers, coadministration of the HIV protease inhibitor saquinavir, also a CYP3A4 inhibitor, at steady state (1200 mg tid) with VIAGRA (100 mg single dose) resulted in a 140% increase in sildenafil C max and a 210% increase in sildenafil AUC. VIAGRA had no effect on saquinavir pharmacokinetics.
